By checking the box below I confirm I am 18 or older, I am signing for myself as the participating player, and:
- Assumption of risk. Soccer and casual adult pickup carry inherent risks (falls, collisions, contact with the field, ball, or other players, and strain or overuse injury). I am in adequate physical condition to take part, and I voluntarily accept these risks for myself.
- Medical authorization. If an injury or emergency occurs, I authorize The Stable and Frontrunner Fieldhouse staff to administer basic first aid and, if needed, to obtain emergency medical care for me at the nearest facility. I am responsible for any costs of such treatment.
- Photo & media release. I grant The Stable permission to photograph me during sessions for marketing and social media. (Opt out by emailing info@thestablealedo.com after signing up.)
- Release of liability. I release The Stable, Aledo Soccer Club, Frontrunner Fieldhouse, and their staff from liability for injury, loss, or damage during any session, except where caused by gross negligence.
- Emergency contact. The phone number on this form is a number I can be reached on, and I will tell staff on arrival of any medical condition, allergy, or injury they should know about.
